CSA® Laboratory Fee - $200
- Includes filing and sending of clinical report
- Clinical report contains two independent measurements of semen sample, and complete analysis of semen data
Shipping via Federal Express - $150
- Samples are shipped in dry liquid nitrogen (LN2) shipper tanks that we can provide either at ambient or pre-chilled temperatures. The round trip cost for a dry shipper in the continental United States is $150.00. Shipping costs to other destinations may vary. For example, round trip shipping to Canada costs approximately $300 US. Note that our dry shippers can hold up to ~ 50 samples frozen in 1-2 ml cryovials. Therefore, with more samples, the cost of shipping can be shared.
- If the Home Collection Kit has been shipped to you and if for any unforeseen circumstance, you would need to cancel, please notify us immediately. We will do our best to cancel the shipment with Federal Express but it may be impossible to do so. The patient will still be responsible for shipping charges. We apologize in advance if this pertains to you.
